From first phone call to first visit
Arranging care for yourself or someone you love can feel like a lot. Here's what happens at each stage, so you know what's coming.
Get in touch
Call us or send an email, and we'll ring you back. We'll ask a few questions about what's happening and what kind of help you're looking for. If you're a social worker, discharge team or other professional making a referral, let us know and we'll take it from there.
A visit at home
Someone from our office will arrange a time to come and see you. We'll talk about your health, your daily routine, what you can manage yourself and where you'd like a hand. We'll also check the home for anything that could make care less safe, like trip hazards or equipment that's needed.
Your care plan
We write down what we agreed: what support you'll get, when, and how you like things done. You'll get a copy, and a copy stays in your home for your carers. If you want family involved, we'll include them.
Meeting your carers
We match you with a small team of carers and aim to keep them the same, so you're not explaining yourself to a new face every day. Where we can, we'll introduce them before care starts.
Checking in as things change
We review your care regularly, and any time your needs change, for example after a fall or a hospital stay. Our office team will also ring to ask how things are going. If you want something done differently, just say.

A copy of your care plan stays in your home, so you and your family can see exactly what's been agreed.
Families are welcome
With your permission, we'll keep your family updated and involve them in decisions. You choose who we can talk to.
Your decisions come first
We always ask for consent. Where someone can't make a particular decision, we follow the Mental Capacity Act and act in their best interests, with the people who know them.
Help at any hour
Outside office hours, our on-call team can be reached on the same number if something needs sorting.
